For Potential Clients

• Initial Inquiry:​ Simply complete our online inquiry form or call our intake line to speak directly with one of our program coordinators.

​

• Secure Information Submission: You’ll be asked to provide basic personal and health information. All data is securely managed through our HIPAA-compliant system, CarePatron, ensuring your records are always kept confidential.

​

• Review & Enrollment: After submission, our team will review your information and contact you to schedule an initial consultation. Once eligibility is confirmed, you’ll receive detailed instructions on enrolling in your chosen program.

For Referral Partners

​

• Referral Submission via FindHelp: We partner with community organizations and referral networks to reach those in need. Referrals can be submitted by completing the online referral form through FindHelp.  Simply provide the client’s contact details and relevant information regarding their needs.

• Confidential & Compliant Handling: All referrals submitted through FindHelp are processed with strict confidentiality and in full compliance with HIPAA regulations. Our intake team promptly reviews each referral and contacts the potential client to begin enrollment.
